The Prescription Drug Plan carrier for 2024 is Express Scripts.
- If enrolled in a Medical Plan you will also be enrolled in the Prescription Drug Plan.
- Prescription drugs are not subject to a deductible.
- Prescription drug copays count towards your annual out of pocket max. Out of pocket max totals are different for each Medical/Rx plan.
- Both the $500 Premier Access Plus Plan and the $1000 Premier Access Plan offer lower copays if filled at the following AMHS outpatient pharmacies: Saratoga Hospital, AMC or Glens Falls Hospital.
Coverage is effective the 1st of the month following date of hire or status change date. If enrolling during annual open enrollment, coverage begins January 1st.
Eligibility - Part time and full time employees budgeted at a minimum of 36 hours for 12 hr employees and 37.5 for all other employees per pay period.
