*Once enrolled the employee must remain in the plan for the entire plan (calendar) year.
Eligibility
- Part time and full time employees budgeted at a minimum of 36 hours for 12-hour employees and 37.5 for all other employees per pay period.
- Employees may enroll within 31 days of date of hire or status change or during annual open enrollment period in November for coverage beginning January 1.
