If enrolled in a Medical Plan you will also be enrolled in the Prescription Drug Plan.
The Medical Plan is administered by CDPHP.
Coverage is effective the 1st of the month following date of hire or status change date. If enrolling during annual open enrollment, coverage begins January 1st.
Eligibility - Part time and full time employees budgeted at a minimum of 36 hours for 12 hr employees and 37.5 for all other employees per pay period.

You have two choices for medical coverage:
- Premier Access Plus ($500) Plan
- Premier Access ($1,000)
Both plans have two tiers of coverage – the Albany Med Health System Network (Tier 1) and CDPHP In-Network (Tier 2). Your out-of-pocket costs are less when you receive care within the Albany Med Health System Network (no cost, in many cases) Preventive care is covered at 100%.
Please note, the Albany Med Health System Network includes providers from: Saratoga Hospital, Glens Falls Hospital, Albany Medical Center, Columbia Memorial Hospital, and The Visiting Nurses Association.
