Eligibilty
- Can enroll at any time. Coverage start date is dependent on the date of enrollment.
- Part time and full time employees budgeted at a minimum of 36 hours (12 hour employees) and at least 37.5 for all other employees can pay through payroll deduction.
- For all other employees, a direct billing option is available.
