Health/Rx Plan

Saratoga Hospital’s health plan is an Exclusive Provider Organization (EPO) providing benefits through a domestic network of Saratoga Hospital facilities and providers and a non-domestic local and national network of providers and facilities. The plan is administered by CDPHP who also administers our prescription benefit.

Coverage is effective the 1st of the month following date of hire.

Plan provides Domestic Partner Benefits.

Eligibility

  • Full time employees budgeted at a minimum of 72 hours bi-weekly for 12 hr employees and 75 for all other employees per pay period. Part time employees budgeted at a minimum of 36 hours bi-weekly for 12 hr employees and 37.5 for all other employees per pay period.

New for 2021

  • Albany Med Health System Affiliate Network
    • We are pleased to announce that starting January 1, 2021, a new Albany Med Health System Affiliate Network will be included as part of the Hospital’s insurance plan through CDPHP.  This new tier will allow you to seek medical care at reduced cost from Albany Med and the other affiliates: Columbia Memorial Health and Glens Falls Hospital (Glens Falls est. to begin 2nd quarter 2021).  We will continue to offer the courtesy discount to all eligible employees through the growing Saratoga Hospital Domestic Network as we have in the past.

  • NYS Autism Mandate
    • Following NYS Autism Mandate to include coverage for the screening, diagnosis and treatment of autism spectrum disorder.

Prescription Insurance

  • Prescription plan is administered through CDPHP.  Co-pay structure is as follows:
    • $10 generic
    • $40 brand and non preferred generic
    • $55 non-formulary brand
    • Specialty drug (Tier 4) is 25% coinsurance after deductible (max of $150 for 30 day supply)
    • Specialty drug (Tier 5) is 37.5% coinsurance after deductible (max of $150 for 30 day supply)
  • The mail order option is available through CVS Caremark and offers a 90 day supply for a co-pay of 2.5 times the 30 day co-pay cost.    
  • CDPHP also provides a program called Rx for Less that provides significant savings on certain medications with a cost as low as $1.00 per script for many medications. Other medications may have a higher cost but still less than our co-pay amount. More information on the RX for Less Flyer.
