- $250 Deductible Health/Rx Plan
- Primary Care office visit copay - changes from $15 to $10
- $500 Deductible Health/Rx Plan
- Primary Care office visit copay - changes from $15 to $10
- $250 Deductible Health/Rx Plan
- For both the $250 and $500 Deductible Health/Rx plans
- CDPHP In-Network Emergency Room Copay - changes from $150 to $100
- This tier is only available to those enrolled in the Saratoga Hospital health/Rx plan

- Albany Med Health System Affiliate Network
- Reminder: The Albany Med Health System Affiliate Network is included as part of the Hospital’s insurance plan through CDPHP. This tier allows you to seek medical care at reduced cost from Albany Med and the other affiliates: Columbia Memorial Health and Glens Falls Hospital. The courtesy discount is still offered to all eligible employees through the growing Saratoga Hospital Domestic Network.
