Form 1: I-9 Document 2013
Form Details: All U.S. employers must complete and retain a Form I-9 for each individual they hire for employment in the United States. We must examine your employment eligibility and identity and document this information on the I-9 form. The list of acceptable documents can be found on the last page of the I-9 document.
Form 2: Employee Data Sheet
Form Details: This information is required to verify accuracy of previously provided information and to ensure accuracy of our employee database. This is also required for IRS reporting, maintenance of employee records and data, and for processing of benefits information.
Form 3: State Tax Form
Form Details: These are personal state tax withholding forms; detailed instructions are included with each form.
Form 4: Federal Tax Form
Form Details: Personal federal tax withholding form; detailed instructions are included.
Form Details: Provides medical history that the Employee Health provider will review with you to help determine whether you are medically able to perform the duties for the position that has been offered to you. The information will not be released to any unauthorized people.
Form 6: Vehicle Registration Form
Form Details: Provides details to our Public Safety Department to issue a parking permit(s) for your vehicle(s).
